Pre-Kindergarten Application Form

If you have a child who is currently or will be four (4) years old by September 1st, 2021, you can now fill out an application for Pre-Kindergarten.

 

We ask that families who intend to apply for our  Pre-Kindergarten program for the 2021-22 School Year complete our Pre-Kindergarten Application Form

 

After completing the Pre-Kindergarten Application Form, you can expect to be contacted if your child is selected for our Pre-K program.

If you have a child who is currently or will be five (5) years old by September 1st, 2021, it is time to register for Kindergarten! 

 

We ask that families who intend to enroll their child in Kindergarten for the 2021-22 School Year complete our Kindergarten Pre-registration Form

 

After completing the Kindergarten Pre-registration Form, you can expect to be contacted by an enrollment official for further details.   

 

Documents you will need to complete for enrollment;

  • Child's Birth Certificate
  • Parents/Guardians Driver's License or Photo ID
  • Copy of child's updated immunization records/dates (from a physician)
  • Proof of Residency (ex. - Utility bill with physical address within 30 days of date of enrollment)

Welcome to the Jefferson-Morgan School District. This enrollment page provides you with information to help you start the process of registering your child in one of our schools. Please complete the attached forms and bring them with you to the District Administration Office located at the front of the Middle/High School. You must call to schedule an appointment to enroll your child(ren). 

In addition to the forms on this page, the following items MUST be presented at the time of registration:

•Child’s Birth certificate
•Parents/Guardians Driver’s License or Photo ID
•Copy of a child’s updated immunization records/dates (from a physician) 
•Proof of residency - (ex. - Utility bill with physical address within 30 days of enrollment date)

 

According to district policy, a student will not be enrolled until all information has been presented to the district registrar.
